34-5. Inspection Requirements.

Equipment Inspection

  • Conduct documented inspections of equipment prior to work, ensuring alignment with manufacturer’s guidelines.
  • Verify equipment's condition and intrinsic safety, especially under demanding conditions.
  • Focus inspections on atmospheric and monitoring devices, ventilation, PPE, fall protection, lighting, access/egress aids, barriers, rescue and communication tools.

Documentation and Compliance

  • Document calibration details for atmospheric testing and monitoring equipment.
  • Ensure that personal protective equipment (PPE), fall protection, and lighting meet the standards detailed in the respective chapters.
  • Review and adhere to requirements for entry and rescue equipment, ladders, barriers, and other essential tools.

34-6. Activity Hazard Analysis (AHA) Requirements.

  • Create Activity Hazard Analyses (AHAs) for every confined space entry, using guidelines from specific paragraphs.
  • Include all possible hazards like mechanical, electrical, physical, chemical, atmospheric, biological, psychological, etc., in the AHAs.
  • Update the AHA if the Permit-Required Confined Spaces (PRCS) have new entry procedures or are reclassified, using forms like ENG Form 6204 or ENG Form 6210 for guidance.
  • Make sure to list all personnel trained for confined space rescue in the AHAs.
