Incident Investigation Criteria

To learn from both non-injury and injury incidents, and help prevent similar incidents, the SEMS program should establish procedures for investing all incidents.

Fires and explosions on an oil rig require immediate BSEE notification.

According to the SEMS rule, incidents must be investigated if facility management or BSEE determines they possess the potential for serious safety or environmental consequences. However, in a "world-class" SEMS program, we believe that the operator/contractor should investigate ALL incidents, regardless of severity, to determine root causes.

Incident investigations should be initiated as promptly as possible, with due regard for the necessity of securing the incident scene and protecting people and the environment.

Incident investigations should be conducted by personnel knowledgeable in the process involved, investigation techniques, and other specialties that are relevant or necessary.

The following types of incidents require immediate BSEE notification:

  • fatalities;
  • injuries that require evacuation of the injured person;
  • loss of well control;
  • fires and explosions;
  • collisions that result in property or equipment damage of more than $25,000;
  • incidents involving structural damage to an OCS facility;
  • incidents involving crane operations; and incidents involving damage to safety systems and equipment.
