28.B.08 Deficiencies.

If any deficiency is identified on the LHE, an immediate determination must be made by the CP as to whether the deficiency constitutes a hazard.

Hoisting equipment must be inspected.
  1. If the deficiency constitutes a hazard, the LHE must be removed from service until the deficiency has been corrected.
  2. The operator must be responsible for the operations under his/her's direct control. Whenever there is any doubt as to safety, the operator must have the authority to stop and refuse to handle loads until safety has been assured.

28.B.09 A Qualified Rigger must inspect the rigging prior to each shift.

28.B.10 The headache ball, hook, or load must not be used to transport personnel.

28.B.11 LHE may be used to hoist employees on a personnel platform when all applicable provisions of Section 16.T have been met.

28.B.12 Safety latches on hooks must not be deactivated or made inoperable.

28.B.13 Structural steel assembly.

  1. Structural stability must be maintained at all times during the erection process.
  2. The following additional requirements must apply for multi-story structures:
    • (1) The permanent floors must be installed as the erection of structural members progresses, and there must be not more than eight stories between the erection floor and the upper-most permanent floor, except where the structural integrity is maintained as a result of the design.
    • (2) At no time must there be more than four floors or 48 ft (14.6 m), whichever is less, of unfinished bolting or welding above the foundation or uppermost permanently secured floor, except where the structural integrity is maintained as a result of the design.
