  • prescription labels
  • patient photos or videos

Wrongful Disclosure of PHI

If you notice an unauthorized PHI disclosure:

  1. Approach the person involved and explain your concerns based on what you observed or heard.
  2. Immediately discuss the situation with your supervisor.

In case of an unauthorized PHI disclosure on your part:

  1. Document the details:
    1. Patient whose PHI was disclosed
    2. Disclosure method
    3. Recipient of the information
    4. Date and time of the incident
    5. Corrective actions taken
  2. Notify your supervisor right away.
