Documenting the Report
Documenting the hazard assessment, analysis, and evaluation should take the form of a formal written certification that includes the following information:
- The workplace that was evaluated.
- The name of the person who did the assessment.
- The date when the assessment was done.
- The name or type of the document that proves the assessment was completed.
If the person conducting the hazard assessment, analysis, and evaluation discovers that hazards requiring PPE are present, or likely to be present, then management must:
- Choose the right PPE to protect employees from the identified hazards.
- Tell each employee about the PPE they need to use.
- Make sure the PPE fits each employee correctly.
