Every organization has a safety management system. In fact, you cannot NOT have a safety management system (SMS).
Remember the principle "you reap what you sow?" It means that future consequences are shaped by past actions, and it applies to the development of all systems. An SMS will produce only what it is designed and implemented to produce. It can't produce anything else.
For instance, if an SMS has long-term outcomes like poor employee safety performance and high accident rates, it indicates that the system is failing. These outputs occur because the SMS has been designed and implemented in a way that produces them. To fix the SMS, it is important to conduct a cause analysis, which is the topic of our next module.
