12-6. Activity Hazard Analysis (AHA) Requirements.

Development of AHAs and HIPs

  • Create AHAs as per guidelines in paragraphs 1-6 or 2-6, as required.
  • Ensure AHAs involving HEC include a Hazard Isolation Procedure (HIP), specifically when controlling hazardous energy.
  • For previously accepted AHAs, submit only the HIP for review.

Review Process

  • Review the HIP with an Authorized Individual to confirm its validity and check for any changes in configuration.

Approval and Acceptance

  • Both the AHA and the HIP must be provided to and approved by the KO or COR before starting the activities.
