22-6. Activity Hazard Analysis (AHA) Requirements.

Development of Activity Hazard Analysis (AHA)

  • Workers performing the work must prepare the AHA according to paragraphs 1-6 or 2-6, as applicable.
  • The AHA must cover, if relevant, site conditions, types and dimensions of scaffolding, anticipated loads, and the CP's name for Scaffolding.
  • Include methods for marking/barricading, steps for installation and removal, fall protection measures, procedures when working over water, fire protection strategies, and any additional hazards.

Review of AHA

  • The Competent Person (CP) must review the AHA with the entire crew immediately before work begins.
  • If certain elements are not applicable, the AHA must include a justification for their exclusion.

22-7. Minimum Plan Requirements.

Scaffold Work Plan Requirements

  • A Scaffold Work Plan must be prepared for: tube and coupler scaffolds, supported scaffolds with a height to base width ratio over four to one, suspended scaffolds, and scaffolds needing tenting due to weather conditions.

Content of the Scaffold Work Plan

  • The plan must include a detailed task description, specifying scaffold type, dimensions, layout, access and egress methods, marking or barricading methods, site conditions, and bracing/tiebacks if necessary.
  • It must outline anticipated hazards, contain a Fall Protection and Prevention Plan if fall protection is necessary, and specify training/qualification for personnel as per paragraph 22-3.
  • Roles and responsibilities of personnel must be clearly defined (refer to paragraph 22-4), and the plan must detail inspection and oversight methods. If an engineer's design is required by 29 CFR 1926 Subpart L, the plan must include the signed RPE design.
