Investigations

Offshore operators must immediately notify BSEE of the following types of incidents:

  • fatalities;
  • injuries that require evacuation of the injured person;
  • loss of well control;
  • fires and explosions;
  • collisions that result in property or equipment damage of more than $25,000;
In-depth investigations occur as a result of serious incidents.
  • incidents involving structural damage to an Outer Continental Shelf (OCS) facility;
  • incidents involving crane operations; and
  • incidents involving damage to safety systems and equipment.

Should a safety or environmental incident occur, the BSEE has a duty to investigate and determine the causal elements/factors and the appropriate corrective actions. The implications of investigation findings may apply to you, the operator, contractors, subcontractors, and may also may extend to industry-wide practices. After-action Analysis

Once BSEE has initiated an enforcement action, after-action analysis is a key element of it's enforcement program and is critical to a risk-based approach. After-action analysis helps identify areas for safety and environmental protection improvements throughout operations on the Outer Continental Shelf.
