2-7. Minimum Plan Requirements.

General Plan Requirements

  • Plans must be developed by each contractor as required by the work being performed.
  • All plans and SOH-related submittals must be submitted as part of the APP to the KO or COR for review and acceptance.
  • Work cannot begin until the plan or other SOH-related submittals are reviewed and accepted by the KO or COR.

Accident Prevention Plan (APP)

  • The APP must be written in English, detail specific work and hazards, and follow ENG Form 6293.
  • Include all known information and update the APP with project-specific details as they become available.
  • The SSHO must conduct ongoing evaluations and updates of the APP. Changes must be reviewed and approved by the contractor and submitted to the KO or COR for acceptance.

Emergency Plan

  • Plans must include evacuation procedures, routes, rally points, and identification of critical plant operations.
  • Onsite emergency planning must be coordinated with off-site support and documented.
  • Develop and test emergency alert systems to alert all persons likely to be affected and summon emergency responders.
